The US Marine Corps
Article Abstract:
The Bell Boeing MV-22B Osprey accident in Apr 2000 has been attributed by the US Marine Corps to human factors. The crew of the accompanying Osprey, which was the lead aircraft in the exercise, has been subjected to 'administrative action' by the Marine Corps. After analyzing the combination of events, the investigators concluded that the crew may have been unaware of the vortex ring state. Had they known, it might have been possible to partially recover and undertake a heavy landing.
