Hospitals in Philadelphia and Pittsburgh have merged in an effort to ward off pressure by dominant insurers to discount charges. The hospitals are struggling to maintain profits while competing with two insurers holding 80% of the managed care market in one city and a single insurer monopolizing the market in the other. Ratings for the hospitals were recently downgraded.

Hospitals are using contract services to save money, provide specialized services, increase competitiveness and to establish new services, according to a survey by Chicago, IL-based AHPI Custom Research. A survey of 778 hospital chief executives throughout the US claim that they contract for emergency, physical therapy, anesthesiology and speech pathology most often.
