Economic incentives, health status and health services utilisation
Article Abstract:
People on low incomes who have free access to medical care use medical services more than those who pay for such care, after other factors have been taken into account, according to a study of Irish households. An implication of this finding is that the public could be encouraged to use primary health care rather than hospital care, which costs more, through making primary care free. Further work needs to be carried out on the effectiveness of primary care in order to assess whether this would be appropriate.

Agency in health care: getting beyond first principles
Article Abstract:
There is a need to carry out research on the utility functions of doctors and patients in order to design systems which include incentives for doctors to take patients' preferences into account when making decisions. There is also a need to carry out more work on devising appropriate pay schemes for doctors who operate in a market where they have easier access to information than their clients. This area of optimal systems of renumeration has so far been neglected by health economists.
