Hospitals, because of financial constraints, are exercising care in hiring consultants. Some steps in evaluating consultants are outlined. For instance, consultants should be tested on short-term, limited projects before engaging them for extensive services. Also, a consultant who makes a low bid may be too busy to devote the required time to a project. In addition to having experience in the designated field, the candidate must have a personality acceptable to the organization's staff. A more expensive candidate may deliver more valuable services.

Surgery delivery systems change as more patients go home the same day
Article Abstract:
Hospitals are re-evaluating their surgery delivery systems in response to a shift from inpatient to outpatient surgery that occurred during the 1980s. Hospital surgical facilities were originally built for inpatient surgery. Hospitals must decide whether to integrate inpatient with outpatient facilities or build free-standing outpatient facilities elsewhere. To be competitive with freestanding surgicenters and physician-owned centers, hospitals must plan their outpatient surgery delivery systems carefully.

Reform debate must address appropriate capital incentives
Article Abstract:
Major health care reform proposals have not fully addressed how hospital spending should be regulated. Many health care leaders believe that the adoption of a national payment system, which incorporates appropriate incentives, will be sufficient to control capital spending. Others believe that regardless of the type of payment system that is finally adopted, a method for monitoring capital spending will be needed.
