QALYs versus HYEs: a reply to Gafni, Birch and Mehrez
Article Abstract:
The derivation of healthy-years equivalents (HYEs) does not require strong assumptions of separability, risk neutrality and zero discounting, assumptions which are needed in deriving quality-adjusted life-years (QALYs). However, the same assumptions need to be made if HYEs are to be used as utility scores. Thus, HYEs as utility scores are no more superior to QALYs in the assessment of health programs versus outcomes. Alternative general utility functions for life years and health states which do not require unattractive assumptions should be formulated.
