A 27-year-old woman with pain in the neck and shoulder and clumsiness of the hand

Article Abstract:

A 27-year-old woman was diagnosed with osteomyelitis in one of the vertebra of her neck with an abscess on the membrane surrounding the spinal cord. Osteomyelitis is an inflammation of the bone marrow and the surrounding bone. She had hit the back of her head against a window and was admitted to the hospital five days later with neck pain that radiated to her left shoulder, and clumsiness in her left hand. X-rays of her spine showed limited movement, and she was given a cervical collar to wear. A magnetic resonance image (MRI) and a CT scan of her neck revealed an abnormality in one of the neck vertebra with a mass on her spinal cord. The patient had had an infected toenail, and infected cuticles on several of her fingers for a period of several months. Tissue removed during surgery on the vertebrae of her neck revealed that the patient had osteomyelitis with an abscess in the membrane surrounding the spinal cord. The osteomyelitis was caused by a Staphylococcus aureus infection that probably developed from the toenail and cuticle infections.

A 54-year-old diabetic woman with pain and swelling of the leg

Article Abstract:

A 54-year-old woman was admitted to a hospital with pain and swelling in her right leg. She had a five-year history of non-insulin dependent, or type 2, diabetes. Tests did not reveal any evidence of blood clots in her leg veins, a condition called deep vein thrombosis. However, an MRI scan of her thigh showed a mass and signs of swelling. A needle biopsy of the mass revealed that she had a muscle infarction, which is an area of dead tissue resulting from a lack of blood. This is not common in diabetics and could be result of arteriosclerosis or changes in blood clotting factors.
