A previously well, 29-year-old woman with a pathologic fracture of the femur

Article Abstract:

A 29-year-old woman admitted to the hospital with a fracture of the right femur was found to have osteomyelitis with an abscess caused by Streptococcus (S.) milleri. Osteomyelitis is an infection of bone. A month before admission, the patient developed thigh pain while crawling under a machine but had a normal x-ray. The pain became worse and on the day of admission she felt a pop in her hip and collapsed in pain. Upon admission, a CT scan and x-ray showed a fracture, destruction of the outer layer of the bone, and a mass in the soft tissue next to the fracture. A biopsy of bone and soft tissue led to the diagnosis. The patient may have bruised her thigh, causing the accumulation of blood which may have become infected with S. milleri from her decayed teeth. The bacterial infection caused bone destruction which resulted in the fracture and abscess. The fracture was surgically stabilized and subsequently healed.

A 41-year-old man with multiple bony lesions and adjacent soft-tissue masses

Article Abstract:

A 41-year-old man was admitted to a hospital with a history of bone pain and fractures that had left him bedridden. A bone scan revealed destructive lesions in his leg and arm bones. He was diagnosed with Erdheim-Chester disease, which is characterized by an infiltration of various organs and tissues by macrophages carrying large amounts of fat. There is evidence that the infiltration may be a type of cancerous process. The disease eventually causes organ failure and death, usually from respiratory distress, lung fibrosis, or heart failure.

A 49-year-old man with the acquired immunodeficiency syndrome and a tibial lesion

Article Abstract:

A 49-year-old male HIV patient was admitted to a hospital with a three-month history of pain in his left knee. He had fractured his left shin bone 12 years earlier. An X-ray of his left leg and a bone scan both showed an abnormality in his left leg close to the knee. A bone biopsy showed that he had a bone infection called osteomyelitis and the organism responsible was Mycobacterium avium complex. This bacterium is a common cause of opportunistic infection in AIDS patients.
