Client-held records in community mental health
Article Abstract:
Tower Hamlets Healthcare NHS Trust in London, England, introduced a client-held record for mental health patients in the community. The record, or care booklet as it was known, was developed in consultation with users and service providers and included sections for personal details, appointments, notes, medication, a care plan, a crisis page and early warning signs. A case study of one patient's use of the care booklet illustrates the conclusion that the booklet improves communication and empowers the patient.

Article Abstract:
A community mental health nurse describes the multidisciplinary support given to a patient with mental health problems. He had been suffering from characteristics associated with the negative features of schizophrenia and was smoking cannabis. It was found that he became overtly psychotic when smoking more cannabis, which is associated with higher degrees of deslusional activity. The method of working with the patient was based on engagement, persuasiion, active treatment and relapse prevention.
