Diabetes Care and Patient-Oriented Outcomes

Article Abstract:

One of the problems in diabetes care is that treatments proved effective in research studies are not quickly adopted by primary care physicians. The Diabetes Control and Complications Trial demonstrated that good blood sugar control could reduce the risk of diabetic complications such as kidney and heart disease. Blood sugar was controlled by multiple insulin injections each day or use of an insulin pump. However, xix years after these results were published, only 10% of patients with type 1 diabetes in the US received this treatment compared with 80% in Germany.

Measuring progress toward achieving hemoglobin [A.sub.1c] goals in diabetes care: Pass/fail or partial credit

Article Abstract:

Hemoglobin [A.sub.1c] (Hb[A.sub.1c]) control is found to be an important outcome for measuring the quality of care provided to diabetic patients. The risks and benefits of performance measures toward achieving Hb[A.sub.1c] goal should be balanced with respect to overtreatment or undertreatment of individual patients in order to maximize the intended population health benefits while minimizing individual patient-level harms and avoiding unintended consequences.
