Global budgets: massive change in pay, practices
Article Abstract:
Speakers at a hearing by the Prospective Payment Assessment Commission opposed the adoption of global budgeting because, among other factors, limits on medical care expenditures would threaten the survival of solo physicians in private practice. According to the American Medical Association, global budgeting would require comprehensive database buildup by government at several levels to determine baselines. This capability would require elaborate computer equipment and years of work. Also, court challenges are inevitable.

Physicians want to collectively bargain; AMA to further the quest
Article Abstract:
The House of Delegates of the American Medical Assn voted in favor of physicians' rights to bargain collectively when signing contracts for managed care. However, the Dept of Justice has declared that such bargaining would violate antitrust law. Physicians' unions, especially in Florida, represent a large percentage of doctors, including residents and interns. The AMA will lobby to remove antitrust restrictions on doctors' rights to negotiate with managed care plans, already granted to podiatrists.

Medicine's litigation center uses courts to press advocacy issues
Article Abstract:
The AMA-State Medical Society Litigation Center is preparing an initiative designed to force states to offer full health care coverage to disabled patients and low income elderly. Since 1970, the Health Care Financing Administration has authorized states to establish specific health care payment rules for the low income patients. The Litigation Center hopes to coordinate state policies with Congressional intent, and preserve patient-physician relationships in the process.
