Leading work teams and improving performance

Article Abstract:

The most effective health care team brings together individuals with different perspectives and talents. Forming an effective team requires determining the team's purpose, evaluating team member's strengths and past experiences, identifying the steps to reach the desired outcome, and eliciting comments on progress from team members. Teams do not always develop in a linear fashion, and leadership needs to anticipate practical and personal obstacles work teams might encounter.

Mission accomplished: a system for discharge planning

Article Abstract:

A six-week pilot study showed hospital efficiency was increased by coordinating admission assessment and discharge planning. A task force of nurses developed an interdisciplinary assessment tool to facilitate patient discharges. Conditions recorded on admission were prioritized for review on discharge. The study evaluated the assessment tool which led to holding discharge rounds twice a week. Interdisciplinary cooperation increased as a result of this process.

Health care focus documentation - more efficient charting

Article Abstract:

Nursing documentation at a hospital was revised to focus on health care topics and reduce charting time. The new system employs a database form completed at admission, a skin assessment and treatment record, a plan of care and problem list, a form for daily patient care, and a discharge flow sheet. The improved forms required interdisciplinary assessment and documented interventions on a single form.
