Systemic vascular resistance index determined by thoracic electrical bioimpedance predicts the risk for maternal hypotension during regional anesthesia for cesarean delivery
Article Abstract:
The likelihood that a pregnant woman will experience low blood pressure from anesthesia during cesarean delivery may be predicted by systolic blood pressure and systemic vascular resistance index. Researchers evaluated the baseline systemic vascular resistance index in 42 women who underwent epidural or spinal anesthesia before cesarean section delivery to determine which women would develop low blood pressure from the anesthesia. The index was calculated with a noninvasive cardiac output monitor to measure stroke volume and cardiac output. Twenty-four women developed low blood pressure and were treated with intravenous fluids and ephedrine. Women who developed low blood pressure under anesthesia were more likely to have higher baseline systemic vascular resistance indices and higher baseline systolic blood pressures.

Post-cesarean section splenic rupture
Article Abstract:
Persistent tachycardia, hypotension, and increased abdominal fluid may suggest a diagnosis of postpartum rupture of the spleen after cesarean section. Tachycardia is accelerated heart action. Labor was induced for a 21-year-old pregnant woman who had elevated serum alpha-fetoprotein levels and complained of reduced fetal movements. Several hours later, the patient complained of abdominal pain and vaginal bleeding. The physician suspected premature detachment of the placenta, and performed an emergency cesarean section. Persistent tachycardia and moderately low blood pressure were noted. A ultrasound scan found an increased amount of fluid in the peritoneal cavity. A laparotomy was performed through the cesarean incision. Major bleeding was observed emerging a tear in the splenic vein, and the spleen was removed.
