'Treat systems, not errors,' experts say

Article Abstract:

The death of a medical writer from a medication error has prompted the medical profession to change the way it deals with these situations. One physician at a 1996 conference on the subject believes punishing doctors for their mistakes leads to a reluctance to report errors. In many cases, the error occurs because of system failures rather than professional incompetence. These system failures should be identified and corrected. The American Medical Association is one of the organizations developing programs to deal with medication errors.

Hospital Collaborative Creates Tools to Help Reduce Medication Errors. (Medical News & Perspectives)

Article Abstract:

A medication safety program in southeastern Pennsylvania could be adopted by other hospitals if it proves to be successful in reducing the number of medication errors. Sixty-five hospitals in the greater Philadelphia area have adopted the Regional Medication Safety Program for Hospitals and it is available to other hospitals through ECRI, a health services research agency in Plymouth Meeting, PA.

Northern Rxposure: US, Canada clash on cross-border medication sales

Article Abstract:

The Canadian government may regulate the sale of Canadian drugs to Americans if Canadians cannot get the same drugs. Many online pharmacies sell Canadian drugs to Americans because they are cheaper than the same drug in the US. This represents 10% of drug revenues for the Canadian pharmaceutical industry, but has caused shortages of drugs in Canada.
