Director jailed in landmark manslaughter case
Article Abstract:
The managing director of OLL, a UK outdoor activity center, was jailed following a verdict of manslaughter over the deaths of four novice teenagers in Mar 1994. The teenagers were sent on a canoeing trip in the open sea with inadequate equipment and inexperienced instructors. The teenagers were told they should not inflate their lifejackets despite the canoes having capsized. The government does not plan legislation to prevent such as situation from happening again, arguing that 1974 legislation is adequate.

Helicopter safety review 'overdue,' says disaster judge
Article Abstract:
A helicopter crash in the North Sea in Mar 1992 led to 11 deaths. The inquiry which followed recommended that a safety review for helicopter flights should take place. The cause of the deaths was given as pilot error and poor working systems. The accident report said that standby ships could have been better positioned and adverse weather conditions were noted. A review would consider factors like survival suits, lifejackets and search and rescue operations.
