When failure is not an option

Article Abstract:

A study of high-reliability organizations is being conducted by Todd La Porte, Karlene Roberts and Gene Rochlin to uncover the reasons behind the success of organizations that operate highly complex and hazardous technological systems almost without mistakes. The study found that the record of safety of such institutions as the US Navy and the Diablo Canyon nuclear power plant is not due to good luck but to their having learned how to deal effectively with a complex technology. Also basic to the effectiveness of these institutions are a layered organizational structure, effective communication, emphasis on active learning and the non-punishment of employees who made mistakes when trying to make the right thing.
