Data reveal that a structured implicit review of medical records at the time of resident's referral will minimize inappropriate transfers.

Selecting target conditions for quality of care improvement in vulnerable older adults
Article Abstract:
Selecting target conditions for quality-of-care improvement in vulnerable older adults is discussed. A study involving discussion and two rounds of ranking of conditions by a panel of geriatric clinical experts who had studied literature reviews was carried out and 21 conditions were selected as targets. The conditions selected are prevalent in the group and probably account for 50%+ of the hospital/ambulatory care given to the group.

Adherence to pressure ulcer prevention guidelines: implications for nursing home quality
Article Abstract:
A study has shown that overall adherence of nursing homes (NHs) to pressure ulcer (PU) prevention guidelines is relatively low, and large variations exist between homes in adherence to many recommendations. A review of NH medical records was carried out to study overall use of PU prevention guidelines and variation in rates of use by NHs in various geographical locations.
