Culture specific implications for decline in ADL and IADL
Article Abstract:
Culture-specific implications for decline in activities of daily living (ADL) and instrumental activities of daily living (IADL.) are discussed. Choice of instrument for a study examining predictors for functional decline in ADLs as well as in IADLs in a 3-yr observational study of nondisabled older Japanese living in a semi-rural community used an instrument that seems to be a matter of local convention more than based on a certain scientific approach. The items in the Tokyo Metropolitan Inst. of Gerontology Index of Competence are analogous to internationally accepted models. Looking at the study shows that geriatric assessment is important everywhere, but that debility and functional capability are often culture-and individual-specific. Certain interventions may be more or less powerful in different ethnic and socioeconomic groups in different parts of the world.

A new twist in the J-shape curve?
Article Abstract:
Older adults with systolic blood pressure (SBP) lower than 125 mm Hg have been found to have almost twice the risk of all-cause mortality than those with SBP 125-134 mm Hg. A J-shape curve would seem to be representative for systolic BP. The low-SBP group has a greater likelihood of mortality in the last 2 years of follow-up and the highest SBP group has the most mortality in the first 2 years of follow-up. Low SBP seems not to indicate current illness. Hazards may accompany overzealous treatment of hypercholesterolemia or hyperglycemia.
