The US Customs has set standards for container security devices mandated by the US Congress. Requirements include detection of door openings after arming, use of common and non-proprietary radio frequencies, and allowing monitoring throughout the journey. The agency wants a maximum overall failure and false alarm rate of 4%.

Unsafe sampling method
Article Abstract:
A cargo surveyor was injured in an explosion aboard a vessel loaded with benzene. The surveyor was taking samples of the cargo prior to the explosion. The accident could have been prevented had the surveyor known about proper sampling methods and equipment.

Fire extinguisher explosion
Article Abstract:
A ship crew member was injured when an old and badly corroded fire extinguisher exploded while he was clearing debris from the engine room. Fire extinguishers should be serviced regularly and not placed in damp areas.
