Pathogenesis of NSAID gastropathy: are neutrophils the culprits?
Article Abstract:
Nonsteroidal anti-inflammatory drugs (NSAIDs) promote the adherence of neutrophil leukocytes to the mucosal wall of the gastrointestinal tract. The neutrophils reduce mucosal blood flow and induce production of proteases which irritate the vascular endothelia and cause ulcers. NSAID-induced gastric ulceration poses some difficulties because it does not produce very evident symptoms and most anti-ulcer medications are ineffective against it. The drug therapy that proves effective in treating NSAID ulcerations is the administration of prostaglandins which inhibit neutrophil activity.
