Home-based cardiac rehabilitation and lifestyle modification: the MULTIFIT model

Article Abstract:

Patients can benefit from home-based rehabilitation which emphasizes exercise, education, and counseling for cardiac patients. MULTIFIT, a model for nursing case management, offers a delivery method which functions efficiently under a managed care plan and meets the criteria for patient surveillance and cardiovascular risk reduction. This model has been successful for Kaiser Permanente's Medical Care Program as well as cost- effective for patients.

Developing a cardiac catheterization education program

Article Abstract:

An education program designed for patients undergoing cardiac catheterization will help reduce patient anxiety. Though the procedure is considered routine by many health care providers, the average patient will not be well-informed about the operation and may benefit greatly from additional information. Health care providers should consider the needs of the patient and develop a well-organized program which covers all aspects of the procedure.

Respiratory care of older adults after cardiac surgery

Article Abstract:

A model is presented for evaluating the lung function of at-risk elderly patients before surgery and individualizing patient care following surgery to maximize independent lung function. These measures can help reduce the need for ventilator support after surgery. The patient's individual lung health, other complicating health conditions, and mental health can all influence post-surgical ventilator support needs.
