Thoughts on the future of Medicare

Article Abstract:

Medicare spending is the primary concern of legislators in the 1995 budget debate. Proposals to reduce financial protection and fragment the Medicare risk pool should be considered as retrenchment in social policy. An enforceable cap in Medicare spending affects budgets for managed care and fee-for-service plans. This essentially transforms Medicare from a defined benefit program to a defined contribution program. Combining such caps with competition in the health care industry puts beneficiaries at a new risk that will undermine the social insurance provided by the Medicare program.

Federal dollars and state flexibility: the debate over Medicaid's future

Article Abstract:

The rising cost of Medicaid has given rise to debates between the federal and state governments over the responsibility for the program's structure and costs. The debates focus on whether Medicaid should remain an entitlement program solely funded by the federal government or be converted to a block grant giving state governments control over the program with a cap on federal funding. The outcome of these debates will affect the future of health care in the country.

Home and community-based waivers for disabled adults: program versus selection effects

Article Abstract:

A study examining the changes in expenditures from participation of disabled adults in a Medicaid home and community-based waiver program is presented. The study analyzes whether the changes in expenditures are due to selection effect or program effect. The study is based on the data from North Carolina Medicaid claims for the period from 1998 to 2000.
