A 35-year-old man with epigastric pain

Article Abstract:

The case of a 35-year-old male registered nurse with a long history of abdominal pain illustrates the difficulty of diagnosing and treating gastrointestinal disorders. Mr. H came from an abusive family, used drugs and alcohol since the age of 12, and smoked heavily. He took large amounts of antacids to relieve the pain caused by stress, spicy foods, drugs, and alcohol. Following the diagnosis of a duodenal ulcer, he began taking ranitidine periodically for years with some improvement. However, his symptoms were not typical of peptic ulcer disease (PUD) and he refused further endoscopy to confirm Helicobacter pylori infection, which is often associated with PUD. Without such confirmation, his doctor was reluctant to treat him with antibiotics. His doctor prescribed clarithromycin and omeprazole and also counseled Mr. H. to quit smoking and have a psychiatric evaluation for possible attention-deficit hyperactivity disorder.

Cancer pain

Article Abstract:

The treatment of cancer pain is reviewed. Treatments include drugs, neurosurgery, deep brain stimulation, and intraspinal infusions of opioids. About 80% of all cancer patients experience pain at some time.
