A 52-year-old man with cardiomyopathy and pulmonary disease

Article Abstract:

A 52-year-old man was admitted to Massachusetts General Hospital repeatedly over the course of many years for treatment of progressive cardiomyopathy (heart disease) and chronic lung disease. He had a history of asthma and was a smoker. He also had a history of alcohol abuse and coarctation of the aorta, which was repaired when he was 22. At 37, he had all of the signs and symptoms of dilated cardiomyopathy, which may have been a consequence of his alcohol abuse. At the age of 52, he was admitted in cardiac arrest and died despite CPR treatment. At autopsy, tissue samples from his heart and lungs displayed the characteristic signs of sarcoidosis. This disease is characterized by the formation of granulomas in various organs, and its cause is unknown. Granulomas were also present in this patient's lymph nodes, liver and kidneys.

A 41-year-old man with leg weakness and mediastinal lymphadenopathy

Article Abstract:

A 41-year-old man was admitted to a hospital because of weakness in his legs and difficulty walking. A chest X-ray showed lymph node enlargement and he also had difficulty urinating. An MRI scan showed no abnormalities in his spinal column, but analysis of cerebrospinal fluid indicated that he might have sarcoidosis. This disease can affect any organ but is most common in the lungs. A biopsy of his lung confirmed that he had sarcoidosis. Sarcoidosis in the spine is very unusual.
