A healthy approach

Article Abstract:

A ward for elderly people at Queen's Medical Centre, Nottingham, England has introduced a nutritional care plan to counter malnourishment. Preliminary patient assessment established physical and mental condition. Individualised care included monitoring food intake and weight changes. Meals became social occasions. Records have shown that the weight of malnourished patients has stabilised or increased. Hospital stays should shorten with the improvement in general health.

Development of the link-nurse role in clinical settings

Article Abstract:

Link nurses are being used by many hospitals as a link person between the ward and a clinical nurse specialist. The link nurse attends meetings and relays information back to other nurses on the ward. Clinical specialty subjects include wound care, swallowing, continence, infection control and nutrition. Other possibilities include HIV, paediatric, care of the elderly, diabetes and respiratory.

Article Abstract:

Nursing assessment and documentation is essential to individual patient care. Nurses may influence medical staff by using documentation to substantiate observation. Documentation also facilitates continuity of treatment. Progress may be monitored and recorded on graphs and charts. Original care plans should be evaluated regularly for optimum nursing care.
