A sailor's heartbreak

Article Abstract:

A 24-year-old sailor describes dyspnea in the setting of substantial weight gain, where the degree of weight gain and the time course suggest that this is a subacute process and the differential diagnosis is broad and includes cardiac, pulmonary, hepatic and renal dysfunction. The sailor had evidence of fluid on the pleural and peritoneal spaces and according to the results of chest radiography, pericardial effusion as well.

Emergency duties and deaths from heart disease among firefighters in the United States

Article Abstract:

A study examined duty-specific risks of death from coronary heart disease among on-duty U.S. firefighters between 1994 and 2004. Results showed that certain emergency firefighting duties, particularly fire suppression, were associated with a risk of death from coronary heart disease that was significantly higher than the risk from non-emergency duties.

A person who suffered from severe dyspnea and hemoptysis is described. He had elevated jugular venous pressure, a right ventricular heave, a loud pulmonic second sound and a grade 2 systolic ejection murmur at the left upper sternal border.
