An elderly man with chest pain in a rural emergency department

Article Abstract:

A case study of an elderly white man presenting at a rural emergency department with chest pain is given. The case was a diabetic with a record of poor control and no known drug allergies. Admission tests revealed congestive heart failure. The case declined a cardiology consult against medical advice. Chest pain should always be considered to have cardiac etiology until proven otherwise. Often, observation of at least 12 hours with serial ECGs and cardiac enzymes will be necessary before a cardiac diagnosis can be excluded.

An acute abdomen?

Article Abstract:

A 58-year old man with abdominal bloating, nausea, and recent diarrhea tore the mesentary, the fold attaching the small intestine to the abdominal wall, while pulling weeds. Abdominal swelling and pain increased over several days. A history of heavy smoking and a recent bout with food poisoning delayed a correct diagnosis until surgery was initiated. After the removal of old blood near the tear, the patient recovered uneventfully.
