Buyer beware

Article Abstract:

The issue of GP fundholding has from the very beginning created certain difficulties for UK politicians, who have never been very sure how to react to it. The main problem is that there are very few clear facts about what the impact of GP fundholding has really been. Furthermore, fundholding itself has been subject to constant changes, and GP practices which have converted to fundholding status more recently have tended not to have the strong commitment of the pioneering practices. It remains very difficult to reconcile purchasing on behalf of an individual patient and purchasing for a whole population.

Article Abstract:

The RCN has seen the emergence of bitter conflict over the issue of whether nurses should drop their commitment to not taking industrial action. The RCN itself is mainly concerned with the amount of the salary increase being offered to nurses by the various NHS trusts, while the more militant elements of the profession are focusing on forcing the government to back down over the imposition of local pay. The leadership of the RCN is now coming under severe pressure, especially as its mandate appears weak.
