Accident investigations: how to ask why

Article Abstract:

Post-accident witness interviews help investigators determine circumstances surrounding an accident and contribute to the formulation of preventive measures. However, investigators often lack the knowledge and training needed to be able to conduct interviews effectively. Witness interviews should be conducted as soon as possible after an accident and should be made on a one-on-one basis at the site of the accident itself. Investigators should never intimidate or blame witnesses to encourage them to become more cooperative.

Fight the safety naysayers

Article Abstract:

Companies need to view safety measures as an investment rather than a financial nuisance, and safety professionals need to recognise resistance to initiatives and overcome them by proving safety-performance records. Safety should be linked to job satisfaction with workers having more responsibility and being recognised for achievement.
