Resolving the Gatekeeper Conundrum: What Patients Value in Primary Care and Referrals to Specialists

Article Abstract:

Many people value their primary care physician but may come to distrust the doctor if they feel he or she is preventing them from seeing a specialist. This was the conclusion of a survey of 12,707 adult patients covered by managed care in California. Over 90% said they would always see their primary care physician first and 89% wanted their primary care physician to coordinate all referrals to specialists. However, 23% believed their primary care physician was preventing them from seeing a specialist. These patients were more likely to distrust their doctor.

Selection and exclusion of primary care physicians by managed care organizations

Article Abstract:

Physicians who serve large numbers of uninsured patients may be denied participation in a managed care organization. Researchers surveyed 947 primary care physicians to see how many had been excluded from participation in managed care. Twenty-two percent had been denied a contract or terminated from one with a managed care organization. Those in solo practice were most likely to denied a contract and were those who served many uninsured patients and non-white patients.
