Home health care: clinical pathways and quality integration

Article Abstract:

Clinical pathways can be an important tool for tracking treatment progress, cost-effectiveness, and quality of home health care. Clinical pathways are scientifically based, individualized treatment plans formulated and tracked by all concerned specialists. Shortened forms can help the patient and caregiver track progress and involve them in the healing process. Home health care agencies using clinical pathways may have a competitive edge in the managed care marketplace.

Clinical pathways in subacute care settings

Article Abstract:

There are several components to clinical pathways for patients in extended health care facilities. Clinical pathways are comprehensive individualized care plans. The pathway should specify baseline capabilities and needs, realistic and measurable treatment goals, expected time to achieve each goal, and how treatment will be coordinated among the family and appropriate health professionals. Changes in expected progress should be noted and categorized.

Assessing the patient's caregiver

Article Abstract:

Nurses can help caregivers prepare for home care and identify potential problems by following a three-step process. The process involves first establishing a relationship with the patient and eventual caregiver and identifying future physical needs. This is followed by identifying sources of conflict and laying the groundwork for long-term resolution. The final stage involves preparing for home care and evaluating progress.
