How to avoid being manipulated by patients: notes from the county jail

Article Abstract:

Trusting one's instincts about a patient and maintaining professional detachment are two of the best ways for clinicians to deal with manipulative patients. A psychiatrist who spent some time working as a psychiatric resident in a prison discusses his experiences. The most difficult task was to distinguish between those who actually needed medication and those who wanted the medication. Manipulative techniques used by prisoners included intimidation, begging and detailed descriptions of hallucinations. Practitioners who have strong emotional reactions to patients should trust their judgement and try to discover why they are having those reactions. A practitioner who can remain calm will be able to sort out inconsistencies in medical histories. Physicians should be wary about using placebos, but should instead focus on a thorough patient exam and medical history analysis.

Pneumocystis pneumonia - Los Angeles

Article Abstract:

On June 4, 1981, the Centers for Disease Control published a report about 5 homosexual men with Pneumocystis carinii pneumonia. This infection was common in people with suppressed immune systems, but it was unclear why these men had become infected. News of other cases in New York prompted the CDC to begin active surveillance of this new disease. By 1983, the responsible agent was known to be transmitted via blood and the agent was found to be a virus in 1984. By 1996, AIDS had claimed 343,000 lives in the US and was the leading cause of death in young people.
