Management of small abdominal aortic aneurysms: early surgery vs watchful waiting

Article Abstract:

Early surgery may be more effective than 'watchful waiting' with frequent measurements for most patients with small abdominal aortic aneurysms (AAA) less than five centimeters (cm) in diameter. An AAA is a localized ballooning of the wall of the aortic artery, which could expand further or burst. A study estimated the incidence of rupture or acute expansion among patients with an AAA smaller than 5 cm in diameter. The risk of rupture or acute expansion was estimated as 14 times higher for patients with an AAA with a diameter of 5.0 cm than for those with an AAA with a diameter of 4.0 cm. Early surgery may increase the likelihood of survival in patients with an AAA with a diameter of 4.0 cm. Early surgery is more effective in increasing the survival of younger patients than of older patients with an AAA of 4.0 cm. Watchful waiting may be more effective for patients with a small AAA with no immediate risk of rupturing or expanding.

Surgery vs Orthosis vs Watchful Waiting for Hallux Valgus: A Randomized Controlled Trial

Article Abstract:

An orthosis can provide short-term relief for pain from a bunion, but surgery is the best treatment for achieving long-term relief. This was the conclusion of a study of 209 patients. An orthosis is a specially created shoe insert made from a cast of the patient's foot.
