Medical errors: Where are we now?

Article Abstract:

The Institute of Medicine (IOM) indicated some seven years ago from 2006 that as many as 98,000 people may die in any given year from preventable medical errors. The US President George W. Bush signed the Patient Safety and Quality Improvement Act of 2005 (PSQIA), which helped to create a national patient safety database for healthcare providers to report medical errors to patient safety organizations (PSOs), and already 23 states in the US have established medical error reporting systems.
