Methyldopa hepatoxicity in pregnancy: a case report

Article Abstract:

Methyldopa, a medication used to treat chronic hypertension in pregnancy, can have serious adverse effects on the liver, although it is often prescribed in pregnancy because it is safer than other therapies. A hypertensive woman was switched to methyldopa at 70 days gestation when pregnancy was confirmed. At 92 days gestation she was admitted with abdominal pain, nausea, and jaundice. At 101 days, her condition worsening, methyldopa was discontinued, and she was transferred from her local hospital to a tertiary care center. No other cause could be found for her illness, and methyldopa hepatitis was diagnosed by exclusion. Her liver function returned to normal as did her blood pressure. She was readmitted at 266 days gestation with pain and jaundice. Her doctor had restarted her on methyldopa through a misunderstanding of her diagnosis when her blood pressure rose. Methyldopa is generally safe in pregnancy, but liver function should be monitored by blood tests.
