Process safety: get a clue to avoid repeat accidents

Article Abstract:

Incident investigation of hazardous chemical releases is required under the Occupational Safety and Health Administration's 'Process Safety Management of Highly Hazardous Chemicals, Final Rule, 29 CFR 1910.119.' Management should have written policy as a sign of commitment to process safety investigations. Trained personnel should be involved in the investigation and report writing. The investigation should occur as soon as possible and should emphasize on the causes of the hazardous chemical release. Conclusions and recommendations should then be drawn to prevent recurrence of the accident.

How bad is violence in hospitals?

Article Abstract:

Violence in hospitals is a serious problem in the US. A National Institute for Occupational Safety and Health study shows that between 1980-1990, a total of 522 deaths occurred among health care workers on the job, with 106 of these workers murdered. In response to this problem, the Occupational Safety and Health Administration has issued draft guidelines on workplace violence prevention program for health care workers. The guidelines include work-site analysis, hazard prevention and control, training and education and recordkeeping and evaluation.
