RBRVS: expectations meet reality in first year of payment reform

Article Abstract:

Physicians have several questions regarding relative value scale payment systems (RBRVS) the Medicare payment reform plan instituted to increase payments for primary care. Some physicians, for instance, did not receive increased payments because the Health Care Financing Administration lowered conversion values for medical services. Additionally, few insurance companies have accepted the RBRVS plan. Congress may change several RBRVS reimbursement and other payment procedures.

Nearly half of doctors have electronic claims capability

Article Abstract:

New data from the AMA Center for Health Policy Research indicates that about 50% of all physicians who billed third parties in 1991 used electronic claims systems. The center reports that 80% of radiologists billed claims electronically, but only 21.7% of psychiatrists billed electronically. Consultant Thomas Landauer states that physicians, except those with low patient volumes, should invest in electronic claims billing if they want to stay in business.

RBRVS still creating hassles in Medicare claims processing

Article Abstract:

Problems with Medicare Part B claims processing may account for the drop in physicians' claims volume. Claims volume, which increased by only 3.8% in 1992, may have been affected by rules associated with the resource-based relative value payment system. For example, a 'common working file' may have eliminated many physician claims for Blue Cross and Blue Shield of Kansas.
