Article Abstract:

A case about a real patient who was hospitalized for the evaluation of prolonged fever and hemoptysis is presented in stages to an expert clinician, who responds to the information by sharing his or her opinion regarding the treatment. The case highlights the need for clinicians to consider a patient's history and treatment carefully when evaluating a new problem and greater awareness of the uncommon complication of ablation therapy is needed among the generalist physicians.

Needle in a haystack

Article Abstract:

A case of a 63-year-old man presented to the emergency department because of shortness of breath that had begun the evening before, after he had gone to bed, and worsened progressively, during the night, is illustrated. Only after magnetic resonance angiography and echocardiography showed evidence of diffuse atherosclerosis, the diagnosis of spontaneous atheroembolism was considered and the 'needle in the haystack' was found.

Inhibiting inflammation in rheumatoid arthritis

Article Abstract:

The ability of phosphatidylinositol-3'-kinase gamma (PI3Kgamma) inhibitors to block inflammation and joint destruction in arthritis is described. It is concluded that clinical trials would be the ultimate test of the hypothesis that PI3Kgamma is a therapeutic target for uniquely human diseases such as rheumatoid arthritis.
