The Relationship Between Managed Care Insurance and Use of Lower-Mortality Hospitals for CABG Surgery

Article Abstract:

Many managed care plans in New York State are not using information about the quality of coronary bypass surgery. New York is one of the few states that collects data on mortality rates from coronary bypass surgery from each hospital in the state. This information is then made public so patients and health care plans can choose a hospital with low mortality rates. However, a survey of cardiac surgery centers in the state shows that patients covered by managed care were less likely to be treated at a low-mortality hospital than those in a fee-for-service plan.

Researchers recommend specific actions to create an electronic medical system that can measure the quality of health care delivery. This data could be taken from hospital databases containing information about patient care. This system can be automated to a great extent, but this means using standard data elements and records. It should be secure enough to protect patient confidentiality while still providing useful information. The system should include protocols for sharing this information with other institutions.

Article Abstract:

Doctors, health care organizations, managed care plans, researchers, and funding agencies need to do more to identify underuse of medical care and find ways to prevent it. Underuse is defined as the failure to provide a health care service that would have benefited a particular patient. Minorities and poor people are least likely to receive needed medical care.
