Repeat problems

Article Abstract:

US refiners insist that their facilities are safe, but a potential for disaster lurks in all of them, and investigative reporters who dig into the limited, but intriguing, data available will find stories to pursue on their local refineries. Access to information about how refineries are run leaks out sporadically, but it is the way the US government collects and distributes accident and safety statistics about refineries that really keeps the public in the dark.

Police shootings

Article Abstract:

Many officers in Houston used deadly force and shot at innocent individuals and various facts like time and location of the shooting, race, sex, age, and whether the opponent was mentally ill were checked. Officer shooters were never disciplined when internal investigations showed that there were significant discrepancies in their accounts of the shootings involving their position at the time and whether they had called for a backup prior to a shooting.
