Response to 'A home-based program for the treatment of acute psychosis'

Article Abstract:

Donald Wasylenski et. al. believed that the costs of medical care for mental health patients can be reduced by adopting alternative hospital programs. The Toronto home care program is an example of a capitated project that can be improved by incorporating the benefits of other alternative programs for acute care. The funding of capitated systems is also necessary for the continuing practice of community psychiatry through cheaper programs.

Response to "The role of self-help programs in the rehabilitation of persons with severe mental illness and substance use disorders." (article of Douglas L. Noordsy et al in this issue, p. 71)(Special Section: Community Psychiatric Practice)

Article Abstract:

Mental health practitioners should adopt a flexible approach to substance abuse by mentally ill clients. Studies have shown that some substance use does not necessarily have an adverse effect on the course of the client's illness. Moreover, mentally ill clients are able to identify both positive and negative effects of the substances they use. Thus, mere admonishment by the therapist to avoid all substance abuse may be counter-productive.

The role of psychiatric consultants in supporting residential staff in psychosocial rehabilitation agencies is discussed. Issues include the emphasis on psychodynamic interpretation, development of internal mechanisms for handling problems, a lack of consumer-centeredness, and contextual barriers to service delivery.
