Drug benefits in managed care: seeking ethical guidance from the formulary?

Article Abstract:

Recommendations are discussed to restore the role of the drug formulary as a cost-cutting measure in managed care organizations. In hospitals, the drug formulary limits the use of drugs to those that are the most cost-effective. This approach could be used in managed care, but a 1996 study of six HMOs found that restrictive formulary policies actually increased the cost of care. This may have occurred because of a narrow focus on drug costs rather than overall patient expenditures.

Mediation and managed care

Article Abstract:

Managed care organizations need to develop effective mediation techniques. The rise of managed care has introduced a third party into the traditional physician-patient relationship. This produces a new set of disputes and shifts the balance of power. The interests of all three parties may not coincide. Bioethics consultation can offer a model for successful dispute resolution.

The development of a palliative care program for managed care patients: a case example

An overview is presented on providing palliative care within a managed care setting. Steps include establishing guidelines and principles, obtaining baseline data, setting up a clinical consultation service, identifying outcome measures, and providing information on program status.
