Article Abstract:
Los Angeles County-USC Medical Center, a public hospital system, may shut down under budget pressure unless the county can find $200 million to match federal funds for a Medicaid demonstration project. Even if the matching funds are found, county health services will be reduced significantly. The county has 2.6 million legal, uninsured residents, of whom 2.1 million are employed. Public hospitals around the US face similar predicaments in providing health care to the uninsured. Some public hospitals are forming public-private partnerships to relieve funding difficulties.

Long term contracts between health care agencies and surgical and medical suppliers are reducing supply costs, providing suppliers with additional business and improving benefits for patients. The committed purchasing programs, which often include a per-member purchasing option, are successful with both large and small health care agencies. For example, Group Health Cooperative of Puget Sound expects to save 4% annually, or approximately $480,000, on supplies.
