Helicobacter pylori and the etiology of duodenal ulcer: necessary but not sufficient

Duodenal ulcer (DU), an ulcer of the first part of the small intestine, causes pain, bleeding, perforation, and, occasionally, death. DU is caused by an endocrine tumor in a small proportion (0.1 percent) of cases and by aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs) in other cases (prevalence of DU is 4 percent to 10 percent among long-term users). DU that is not related to these two causes is invariably associated with excess secretion of gastric acid and with chronic antral gastritis (CAG; inflammation of the part of the stomach near the duodenum). In these cases, the microorganism Helicobacter pylori (HP) is almost always found in the antrum. However, infection with HP has not been shown to cause DU or CAG. Risk factors for DU include male gender; family history of DU; cigarette smoking; greater age; negative emotional reactions to life stress; and abnormalities in the secretion of gastric acid and pepsin. Reducing the secretion of acid with drugs is a common method of treatment, as is the elimination of CAG related to HP infection. It is not yet known whether HP and CAG can be permanently eliminated in DU patients in a manner that is also safe and affordable. Until this is known, clinicians should continue to treat DU with drugs that reduce acid secretion and help patients to control risk factors. He believes that when companies let increased production drive their expansion strategy at the price of quality, the firms reduce their capacity to innovate and change. Tiefenbrun is hostile to government regulation, and believes that too much time is spent documenting the requests of government regulators.

Network operators Cellnet and Vodafone have radio base stations to link hand-portable mobile phones with the ordinary system. About 40 companies buy airtime from thenetworks. These companies have contracts with users. The small print of the contract shows whether charging is in 30-second units or full minutes.
