Medication can induce severe ulcerations

Article Abstract:

The case report is presented of a 40-year-old man who developed severe mouth ulcers that had not healed during a six-month period. Each sore lasted about six weeks and was replaced by a new one as it healed. The patient had lost 15 pounds. His physician had prescribed penicillin and acyclovir, which had not been effective, and another physician had applied silver nitrate to the lesions. The patient had undergone extensive diagnostic testing for infection, blood disease, malignancy, and AIDS. He had also consulted a psychiatrist. The lesions were found to be the result of nonspecific inflammation. The patient's medical history included two surgeries of the lower back for degenerative joint disease and one back surgery to remove a metal rod in the back to which an allergy had developed. Allergies also existed to pollen, bee stings, and ibuprofen (a nonsteroidal anti-inflammatory drug). The patient took piroxicam, a nonsteroidal anti-inflammatory drug for chronic lower back pain. The diagnosis of ulcerative stomatitis (sores on and inflammation of the mouth's soft tissues) due to piroxicam was made, and use of the medication was stopped. The patient was told to use a tetracycline elixir and another agent to control pain. The ulcerations disappeared within two weeks and did not recur. Oral ulcers occur in more than 1 percent of patients who take piroxicam. Other diagnoses that were considered are described and discussed. Clinicians are urged to consider the medications taken by patients who have oral disease with no apparent cause. (Consumer Summary produced by Reliance Medical Information, Inc.)
