Pit and fissure sealant application: updating the technique

Article Abstract:

Glass ionomer tooth-surface sealants are inferior to resins. The tooth must be completely above the gum line for successful bonding. The tooth surface must be cleaned. Historically, a prophy cup or bristle brush and pumice have been used. However, cleaning with an explorer and a water jet, toothbrush and toothpaste, and air polishing also work. Widening the fissures with a drill may improve sealant penetration. Either a rubber dam or cotton rolls may be used to isolate the tooth and keep it dry. Each has advantages and disadvantages. Air abrasion is inferior to acid etching and should not be used. Acid preparations come in gel and liquid forms and each has advantages and disadvantages. Fifteen to 20 seconds is adequate etch time. All pits and fissures must be covered, but excess sealant can interfere with proper bite. A successful seal requires sufficient polymerization time. Sealed surfaces should be periodically evaluated and repaired if needed.

Trends in preventive care: caries risk assessment and indications for sealant

Article Abstract:

A comprehensive risk assessment can be used to determine whether a patient needs dental sealant to prevent cavities. Risk factors for cavities include lack of oral hygiene, a cavity-prone diet, low fluoride intake, dry mouth, and a history of cavities.
