Assessment of adherence to renal dosing guidelines in long-term care facilities

Article Abstract:

A cross-sectional chart review for the month 5/99 has been carried out for four long-term care facilities to find out whether dosing guidelines based on creatinine clearance (Ccr) for renally excreted medications are being applied when drugs are prescribed for long-term care residents. It was found that renal function is often overlooked when these medications are prescribed for this population. Approximately one in three prescriptions was found inappropriate for the calculated Ccr for the patient.

Malnutrition in institutionalized seniors: the iatrogenic component

Article Abstract:

A retrospective secondary analysis of nutrient intake and delivery data for 21 consecutive collection days has been carried out at a geriatric teaching facility affiliated with the medical school of the University of Tor onto to give practitioners benchmarks for micronutrient supplementation policies for older residents of long-term care facilities, based on usual energy intake for residents . The traditional approach of developing menus for the facilities using food guides from N orth American governments results in iatrogenic malnutrition so n ew guidelines for this group are needed. They should, in the interim, receive full- sp ectrum vitamin/mineral supplements.

Health care quality, the older patient, and the primary care physician

Article Abstract:

A multidimensional strategy in which the role of the primary care physician is important is needed in prescribing for the elderly. Both overuse and underuse of medications in the elderly should be considered when quality of medical care for this population is discussed. Overprescribing would not necessarily be indicated by a patient's using five or more prescribed drugs. Measurement of overprescription is more complicated.
