Preventing disability in older people with chronic disease: what is a doctor to do?
Article Abstract:
Intervention as preventive medicine can be beneficial, but these benefits are not sufficiently conclusive to warrant restructuring health care system procedures. Effects of intervention should be specified and quantified, and related directly to conditions under treatment. A wide variety of factors may also alter the scope and efficacy of intervention strategies.

Preventing disability and managing chronic illness in frail older adults: a randomized trial of a community-based partnership with primary care
Article Abstract:
Collaboration between communities and primary care providers can improve health care for frail elderly adults. Community-based intervention programs, led by trained medical staff and coordinated with organized medical services, decrease inpatient utilization, improve the management of chronic illness, increase independent functionality and prevent disability.
