The effect of a physician assistant on the hospitalization of nursing home residents
Article Abstract:
Increased nursing home visits by a gerontologist physician assistant (PA) may decrease patient hospital admissions and the number of days nursing home patients require hospitalization. Researchers monitored hospitalizations among residents at a 92-bed nursing home before and after a physician assistant began making regular visits, alternating with attending physicians in caring for residents. Hospital admissions fell 38% after the PA visits began. The annual cost of physician and PA visits increased $22,304, but costs for hospitalization fell $96,043.

Newer psychotropic medication use in nursing home residents
Article Abstract:
Many nursing home residents are being treated with new-generation antipsychotic drugs that have fewer side effects. Researchers analyzed the medical records of 298 nursing home patients referred to a geriatric psychiatry service in 1995-1996. Sixty-nine percent were taking antipsychotic drugs. Antidepressants were used in 61%, and 53% were taking a serotonin reuptake inhibitor. Doses fell within the guidelines proposed by the Health Care Financing Administration in 1990.

Managed primary care of nursing home residents
Article Abstract:
A study to measure rates of hospital use and mortality of nursing home residents whose Medicare HMO primary care came from nurse practitioner-physician teams has been carried out in Southern California. Integrating efforts of physicians, nurse practitioners and nursing home staff can bring about low rates of hospital utilization by nursing home residents. Quality of life and mortality effects should be studied more thoroughly.
