Transitional care of older adults hospitalized with heart failure: a randomized, controlled trial

Article Abstract:

The effectiveness of a transitional care intervention delivered by advanced practice nurses (APN) to elders hospitalized due to heart failure is examined. It was concluded that the transitional care intervention increased the length of time between hospital discharge and readmission or death, reduced total number of rehospitalizations and decreased healthcare costs.

Oral health and the role of the geriatrician

Article Abstract:

This article examines the role of the geriatrician in providing oral health services to older patients. Geriatricians should play a role in managing patients' oral health due to the fact that many elderly patients have limited access to dental care and that oral diseases may contribute to systemic health problems.

Managed care for older people: a primer for the geriatrician

Article Abstract:

The principles of managed care for older patients is discussed. An overview of managed care is provided, including plan types and information on benefits, risks, and other comments. Geriatricians are encouraged to become involved, especially in view of the recently passed Balanced Budget Act of 1997.
