Chronic care clinics: a randomized controlled trial of a new model of primary care for frail older adults
Article Abstract:
A study of chronic care clinics for frail older adults with common geriatric syndromes found the patients were satisfied with the approach, but that improved outcomes for selected syndromes were not seen. A need remains for developing greater systemwide support for managing geriatric syndromes in primary care. Practice improvement research is needed in a rapidly changing delivery system. A randomized controlled trial investigated whether such clinics can provide adequate primary care for frail older adults. The clinic was held for a half day every 3-4 months over 24 months with 5 physicians, and 96 patients were in the study. Control practices with 4 physicians and 73 patients maintained the usual care.
